Human Blood Samples
All human blood samples submitted for research must be accompanied by a signed consent form. If you plan to submit samples, please read the consent form below and sign a copy for each donor. Please send completed pages 5-7 of the form with each sample. The consent form may be sent separately from the samples either by mail, FAX (FAX number 573-884-4100) or e-mail (katzm@health.missouri.edu).
